The latest RCH National Child Health Poll, titled “Hooked Early: Nicotine use among Australian teens”, arrives wrapped in the familiar language of alarm. Teenagers, we are told, are being “hooked early.” Nicotine use is framed as widespread, insidious, and escalating. The implication is clear: Australia is losing control of youth nicotine, and urgent intervention, more enforcement, more restriction, more prohibition, is the only responsible response. But when the poll is read carefully, and when its findings are placed alongside broader national data and recent policy changes, the story becomes far less dramatic and far more troubling in a different way. The real problem is not what the poll shows, but how it is framed, what it omits, and how eagerly it is being used to reinforce a pre-existing narrative.
The poll reports that around one in five teenagers have ever tried a cigarette, just over one in five have ever tried vaping, and smaller proportions report current use. These numbers sound confronting when presented in isolation, but “ever tried” is one of the weakest and most misleading metrics in public health. It captures everything from a single experimental puff to habitual use, yet the report consistently allows this category to bleed into discussions of harm, dependency, and risk. In doing so, it inflates curiosity into crisis. Experimentation among adolescents has existed for as long as nicotine products have existed, and decades of research show that most experimentation does not progress to regular use. The poll does little to distinguish between fleeting experimentation and sustained behaviour, even though only the latter meaningfully drives long-term health outcomes.
This lack of distinction becomes more problematic when the report turns to nicotine dependence. The indicators used are soft, subjective, and easily overinterpreted. Teens reporting that they might find it hard to stop using a product, or that they use it occasionally when stressed, are implicitly categorised as showing signs of addiction. This stretches the concept of dependence well beyond its clinical meaning. True nicotine dependence involves compulsive use, escalation, withdrawal, and persistence despite harm. The poll’s framing collapses that complexity into a broad net that catches almost any engagement at all. The result is an impression of widespread dependency that the data simply does not robustly support.
What makes this framing more concerning is the absence of meaningful context. Australia already has some of the lowest youth and adult smoking rates in the world. Nationally representative surveys consistently show that daily smoking is at historic lows and continues to decline. Those trends do not disappear because a smaller, faster poll highlights experimentation. Yet the RCH report does not meaningfully situate its findings within these long-term declines. Instead, it presents youth nicotine use as a standalone emergency, disconnected from decades of progress and from the broader epidemiological picture.
Even more striking is what the report does not address at all: policy. Over the past two years, Australia has implemented one of the most restrictive vaping regimes in the world, culminating in a near-total ban on consumer vaping products. This policy has fundamentally altered how young people and adults access nicotine, pushing supply into illicit markets and removing lower-risk alternatives from regulated retail settings. Emerging data from independent sources suggests that, following these reforms, smoking among young adults has increased, not decreased, and that vaping and smoking have risen together rather than one replacing the other. None of this context appears in the RCH poll. The data are presented as if youth behaviour exists in a vacuum, untouched by policy decisions that have dramatically reshaped the market.
The timing matters. When a poll highlighting youth nicotine use is released shortly after sweeping prohibitions, and when it omits any discussion of how those prohibitions may be influencing behaviour, it stops being a neutral snapshot and starts functioning as political reinforcement. The findings are easily weaponised to argue that bans must be tightened further, enforcement must be harsher, and dissenting voices must be ignored even when there is growing evidence that prohibition is producing unintended consequences.
Media coverage has largely followed this script. Headlines lift the most alarming statistics, repeat the phrase “hooked early,” and move quickly to calls for tougher controls. Very little attention is paid to the difference between trial and regular use, to the lack of longitudinal evidence showing progression to addiction, or to the broader downward trends in smoking-related harm. The poll becomes a moral signal rather than an analytical tool. Concern replaces comprehension.
This is not to say that youth nicotine use should be ignored or dismissed. It should be taken seriously, proportionately, and honestly. But seriousness requires precision. It requires distinguishing between harm and risk, between experimentation and dependence, and between correlation and causation. It also requires acknowledging that not all nicotine products carry the same level of danger, and that policies which eliminate lower-risk alternatives may inadvertently entrench more harmful behaviours.
The RCH poll does none of this. Instead, it contributes to a growing pattern in Australian tobacco control discourse: selectively emphasising data that supports prohibitionist instincts while sidelining evidence that complicates them. The result is a public conversation driven more by fear than by nuance, more by institutional alignment than by scientific balance.
If this report tells us anything, it is not that Australian teenagers are “hooked early,” but that public health messaging is increasingly hooked on oversimplification. We are repeatedly shown snapshots without trajectories, signals without baselines, and concern without context. That may be effective for headlines and policy justification, but it is a poor substitute for rigorous analysis.
Public health should not be about winning arguments or protecting reputations. It should be about reducing harm in the real world. That goal is not served by treating every instance of experimentation as a crisis, by ignoring policy effects, or by collapsing complex behaviours into alarmist slogans. If we are serious about protecting young people, we need less panic and more honesty, even when the data tells a less politically convenient story.

The questionnaire is not a neutral instrument seeking to understand youth behaviour; it is a set of prompts that systematically inflate prevalence, blur distinctions, and reclassify ordinary adolescent behaviour as pathology.
The most fundamental flaw is the repeated reliance on “ever tried” as a core metric. A single puff of a cigarette, one drag on a vape at a party, or a moment of curiosity months ago is treated as analytically meaningful nicotine use. There is no threshold separating experimentation from engagement, no distinction between a one-off experience and sustained behaviour. In adolescence, a life stage defined by experimentation, this guarantees inflated prevalence by default. The survey design does not merely allow this confusion; it depends on it.
Even when the survey shifts to “current use,” the definitions remain vague. Frequency is poorly specified, meaning someone who used a vape once in the past month can be counted alongside someone using daily. Regular use, occasional use, and incidental use are collapsed into a single category. This makes it impossible to assess risk in any meaningful way, while still producing numbers that sound alarming. The absence of granularity is not a technical oversight; it is a structural choice that maximises perceived harm.
The dependence questions are where the survey becomes especially misleading. Respondents are asked whether they feel tempted to use nicotine, whether they have thought about cutting down, or whether stopping might be difficult. These responses are then framed as “signs of dependence.” But these are not measures of addiction in any clinical or behavioural sense. They are normal cognitive responses to any mildly pleasurable activity. Enjoyment, habit, or ambivalence about stopping are quietly transformed into evidence of dependency, even when no compulsive use, escalation, or harm is demonstrated.
The survey also flattens risk across products. Cigarettes, vapes, and nicotine pouches are treated as morally and medically interchangeable, despite overwhelming evidence that they differ dramatically in harm. Combustible tobacco is uniquely lethal; non-combustible nicotine products are not. By refusing to differentiate meaningfully between them, the survey ensures that any nicotine exposure, however minimal, can be rhetorically linked to the worst possible outcomes.
Motivations for use are framed narrowly as well. Stress, anxiety, social situations, and curiosity are listed, but there is no attempt to explore substitution effects or harm reduction dynamics. The survey never asks whether vaping is replacing smoking, whether experimentation would otherwise have occurred with cigarettes, or whether access to different products shapes behaviour. Harm is assumed to flow in one direction only, and the questions are constructed accordingly.
Equally telling is what the survey does not ask at all. No questions are probing the effects of recent vaping prohibitions, no inquiry into black-market access, no exploration of how supply restrictions alter behaviour. In a regulatory environment defined by prohibition, this omission is extraordinary. Behaviour is measured, but policy is invisible. The result is data that can be used to justify regulation without ever evaluating its consequences.
Taken together, these design choices produce exactly the outcome the report delivers: high “ever use” figures, loosely defined “current use,” and expansive “dependence” indicators that suggest a crisis without ever demonstrating one. The survey does not falsify data, but it steers both respondents and readers toward a predetermined conclusion. By the time the results are published, the narrative has already been locked in.
This is why the poll integrates so smoothly into media panic and political messaging. The survey questions are already aligned with those objectives. They privilege breadth over depth, feelings over behaviour, and implication over evidence. What looks like neutral data collection is, in practice, concern-confirmation.
If public health is meant to illuminate reality rather than dramatise it, this is a problem. Honest research distinguishes experimentation from entrenchment, risk from harm, and policy from observation. The RCH nicotine survey does none of these things. Instead, it produces authoritative-looking data that obscures the distinctions that matter most.
If we want a serious conversation about youth nicotine use, we need better questions, not more alarming answers.

Moral panic over youth vaping/nicotine is really all the tobacco control “mafia” have left to work with. No half-informed person is going to say that vaping is not safer than cigarettes, or that it is the most effective way to quit or it causes harm to bystanders. So they cling to the “what about the kids” argument which is slowly slipping away.
